Rates were obtained by direct standardization of incidence rates in the commercially insured and Medicaid population, by age, sex, and payer, using data from the U.S. Census Bureau, Current Population Survey and Annual Social and Economic Supplements. 95% CIs were calculated using the direct method as outlined by Fay and Feuer (1997) [34]
